Hattiesburg Clinic, P.A. ~ Medical Director – Post Acute Care Opportunity
Hattiesburg Clinic, one of the largest physician-owned multi-specialty groups in the Southeast, is seeking a BE/BC Geriatrician, Internal Medicine or Family Medicine physician to join our staff in a Medical Director role serving patients in area nursing homes and working with the Quality Management team providing post-acute care. The candidate would provide medical house services at community skilled nursing facilities and potential in-home visits, in collaboration with advanced practice providers. Candidates with American Board of Post-Acute and Long-Term Care Medicine Medical Director Certification preferred.
Our Quality Management team includes the Medical Director, 2 advanced practice providers, and 40 registered nurses, who coordinate with 25 board-certified Internal Medicine physicians in our hospital care service program and a transitional team of RN case managers. The goal of the post-acute program is to reduce the SNF length of stay and readmission rates.
