This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Learn more The Associate Chief of Primary Care is responsible for providing clinical leadership to the PACT Teams supporting continuity or special populations clinics, standardizing clinical practice to comply with Primary Care Delivery System and PACT implementation goals and directives. The Associate Chief of Primary Care reports to the Chief of Primary Care Service and works closely with the Nurse Managers to achieve the goals and objectives of Primary Care Service Line, Medical Center, and VISN. The Associate Chief serves in the absence of the Chief of Primary Care Service. Duties and functions of the position include but are not limited to: Responsible for the primary care of assigned patient panel across the continuum of care. Demonstrates knowledge, skills, and abilities to conduct comprehensive health assessments; determine diagnoses and health care needs; and to construct health care plans based on effective therapeutic ability, consultation, patient education and established practice guidelines. Maintains effectiveness in recognizing emergency/acute situations and intervening in an appropriate and effective manner. Documents patients' health care clearly, adequately and accurately as defined by medical records documentation, chronic disease and prevention 'indicators. Supervises clinicians and mid-level Primary Care providers. Maintains clinical and administrative competencies. Provides for the ongoing education of providers by ensuring regular journal clubs and other activities are conducted and attended either on-site or through tele-technologies. Reviews clinical performance monitors with providers and clinical teams and assists work units in developing and implementing quality improvement efforts. Assists in PACT implementation at care sites, including staff development, drafting policy and monitor review and oversight. Prepares written results either for publication and/or for local implementation. Attends assigned committees and required staff meetings.
