The Dir, Ambulatory Care Coord & Value-Based Care Programs provides overall vision, leadership, and direction of the practice-based and adult care management multidisciplinary team. This team consists of nurses, dietitians/diabetes educators and social workers/behavioral counselors who focus on patient and family-centered principals and concepts in the delivery of patient care. The Dir, Ambulatory Care Coord & Value-Based Care Programs collaborates with the Managed Care team to optimize payor strategy and internal initiatives directed at optimizing quality outcomes, cost of care and fiscal incentives.
